Based on interview, observation, and record review the licensee did not comply with the section cited above in that the licensee altered where care and supervision would be provided to preschool age children, without notifying the Department. This resulted in 3 of 6 preschool age classrooms no longer being used by preschool age children without notification. This poses/posed a potential health/safety or personal rights risk to persons in care.
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the review of staff files the LPA was unable to review a current Mandated Reporter certificate for 5. Director stated S5 has not renewed the training.
