Director will submit lead documents to LPA by 11/07/22. Director submitted a maintenance work order on 11/02/22 to replace outlet A and K fixtures. Director will flush outlets and retest. Director states results of retest will be submitted to LPA by 12/02/22. This requirement is not met evidenced by record review of the lab report analysis from Outlet A and K taken on 09/27/22. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care. 8
