Based on record review, the licensee did not comply with the section cited above in that 2/5 children's files, Child 3 [C3] & Child 4 [C4], did not contain a written medical assessment. These files also did not contain an appointment date for the medical assessment of the children or a tuberculosis test within 30 days of the children's admission. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interview and record review, the Licensee did not meet the above regulation which poses an immediate health risk to the children in care. The facility self-reported a child being observed with a vape pen in their possession and believed the vape pen was in the child’s mouth.
