Based on interview, Licensee did not ensure that Child #1's personal rights were protected when Staff #2 took C1 by the hand/wrist, to bring them to the restroom to wash hands. This posed a potential risk to the health, safety, and personal rights of children in care. 8 Also Options will contact Zen Educate to inform them that they no longer wish to have Staff #2 as a substitute for all Options For Learning locations. Emails will be forwarded to LPA by POC date
