Based on LPA's observation, and record review, the licensee did not comply with the section cited above as the Director, Kendra Dedmon and the staff, Marcela Zavala's mandated reporter certification expired which poses a potential health and safety risk to persons in care. The Director's certification expired on 03/16/2020 and staff Zavala's expired on 02/27/2020.
