On June 17, 2026, Child #1 (C1) sustained scratches and bruises to their face and nose, and staff could not explain how, when or where the injuries occurred which poses an immediate health, safety, or personal rights risk to children in care. 8 Facility Representative stated that the agenda, summary of the training and staff sign in sheets will be provided to the San Diego Regional Office, no later than September 2, 2026.
