Based on observation, interview and record review, the licensee did not comply with the section cited above, in that LPA observed 13 children in the 2 year old classroom being supervised by one teacher which poses a potential, safety, or personal rights risk to persons in care.
Based on interview and record review, the licensee did not comply with the section cited above, in that 10 children were transported in a 9 passenger van by Celia Carrizosa exceeding the vehicle's seating capacity which poses an immediate, safety, or personal rights risk to persons in care. 8 Facility Representative stated she will ensure all staff responsible for transportation of children review the video Child Care Centers Operators located on the CCLD and submit a staff attendance sign in sheet and summary of the video, to the SDRO by 10/28/2025.
Based on interview and record review, the licensee did not comply with the section cited above, in that C1 was transported in a 9 passenger van by Celia Carrizosa without a seat belt which poses an immediate, safety, or personal rights risk to persons in care. 8 appropriate retraint system. Facility representative stated that she will submit a a written plan, to the SDRO by 10/28/2025.
Based on interview, the licensee did not comply with the section cited above, in that C1's authorized representative was not notified that on 9/19/25, C1 was transported to the facility in a 9 passenger van without a seat belt which poses an potential, safety, or personal rights risk to persons in care. 8 occurrance of incidents involving their children. Facility Representative stated that she will submit staff signatures as acknowledgement that they have read the regulation and a written plan, to the SDRO by 11/10/2025.
Based on observation, interview, record review, the licensee did not comply with the section cited above in that LPA observed the facility operating beyond the capacity limitations specified on the license in that there were 14 school age children present during the time of inspection which poses an immediate health, safety or personal rights risk to persons in care.
Based on Directors own admission, interviews conducted and records reviewed, the facillity did not comply with the section cited above by having one teacher supervising more than 12 children from 10/23/23 to 11/8/23, which poses an immediate Health, Safety, and Personal Rights risk to persons in care.
Director states some teachers are fully qualified but did not designate a teacher to fill in as the substitue Director. Director states she will talk with the staff that can be designated to be the substitute Director, if needed. Director states she will call them to let them know if they need to be designated. LPA arrived at the facility at 7:45 AM and the Director was not on the premises at the time of this inspection. LPA asked for the substitute Director and LPA was advised the substitute Director was not available as well. Staff did not know who was in charge. Director arrived on or about 8:30 AM. 8 Director states she will send a copy of the letters sent to staff. Director will send in a written statement that she understands the regulation and will comply.
