Based on interview and record review, the licensee did not comply with the section cited above, in that C1 was left unsupervised between the path from the children's restroom to classroom which poses an immediate, safety, or personal rights risk to persons in care. 8 Facility Representative stated that she will retrain staff on supervision procedures and provide a staff attendance sign in sheet, a summary of the training and a plan for supervising children to the San Diego Regional Office, no later than December 4, 2025.
