S2 was observed caring for 9 children alone and S4 was caring for 7 children alone in the classroom. Staff #3 did not have transcripts available for LPA to review. LPA is unable to determine if Staff #3 is a fully qualified teacher. S3 was observed supervising 7 children alone in the classroom. 101238(a) Buildings and Grounds.The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. The requirement is not met as evidence by: Based on LPA’s record review, outlets B and E reported high lead levels. This poses a potential risk to the health of children in care. 1 Director had labeled and covered the outlets with plastic, and tape. Per Director, the facility will use the seven other water faucets, as other water resources for children and staff at the facility. Per director, the children are encourage to bring their water bottle from home.
