During outdoor physical plant inspection, LPA observed the bottom part of a section of the fence surronding the infant play yard rustic, rotten, and with holes. Based on staff interviews it has been determined S1 and S2 yelled at day care... 1 Director stated staff were spoken to and an all staff meeting will be conducted. A copy of the agenda and staff sign ins will be sent to LPA once completed. children. This is an immediate risk to the health and safety of children in care.
Based on LPA’s facility inspection and interviews conducted with 5 staff members it has been determined the sink next to the changing table is in disrepair. This poses a potential health, safety or personal rights risk to persons in care.
