This requirement is not met as 1 The Facility Representative stated she will reach out to the landlord today and will come up with a plan to remove the trash from the premises. The Facility Representative is also required evidenced: LPA observed a large pile of trash in bags which has food waste, diapers and other disposed items. The pile of trash is located in front of the entrance of the facility. This is an immediate risk to the health and safety of children in care. 8 to come up with a plan for a permanent fix to ensure the trash is regularly removed from the premises. POC is due by 01/07/2022 LPA will return to correct the citation.

This requirement is not met as evidenced: 1 Facility Representative will ensure to clean and organize outdoor toys and ensure they are properly stored and sanitized. LPA observed infant toys scattered on the ground of the infant play yard, there were toys in bins covered in dirty rain water. This is an immediate risk to the health and safety of children in care. 8

There shall be a ratio of one teacher for every four infants in attendance. This requirement is not met as evidenced: Facility Representative was in ratio with six infants. She had an individual in the 1 Facility Representative stated that she recently fired two staff from the infant room and is in the process of filling their position. Facility Representative will submit a written plan of correction by POC date of infant room on a working interview which can not count in ratios. This is an immidiate risk to the health and safety of children in care. 8 01/06/2022. LPA will return to correct the violation.

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: This requirement is not met as evidenced: Facility Representative was provided blank LIC9163 forms. She will ensure both individuals are fingerprint cleared by POC date of 01/06/2022 or they can no longer provide care to children. Staff Aleta Pinkney and Shanatta Rivers do not have fingerprint clearances. This is an immediate risk to the health and safety of children in care. An immidiate civil penalty in the amount of $100 is assessed per individual. 8

The written plan is due by 01/06/22. postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university. This requirement is not met as evidenced: Staff #2 did not have proof of ECE units, Staff #3 did not have units related to infant care. 8

The facility will also be deep cleaned.

LPA will return to correct the violation. LPA observed a leaning fence in the play yard which separates the facilities and the neighbors yard.
