At 9:15 AM, Licensing Staff observed two staff leaving the facility with 10 children from preschool and school age program. 1 Licensee will submit a written plan of correction in writing to LPA by POC date of 07/28/2023. Licensing Staff observed the same staff supervising the children at Jack Davis park at 11:58 AM. This is an immediate risk to the health and safety of children in care. 8

(c) To be a fully qualified teacher, a teacher shall have one of the following: (1) Twelve post-secondary semester or equivalent quarter units in early childhood education or child development completed, with passing grades, at an accredited or approved college or 1 Licensee will submit a written plan of correction in writing to LPA by POC date of 07/28/2023. university; and at least six months of work experience in a licensed child care center or comparable group child care program. This requirement is not met as evidenced: LPAs observed two staff leaving the facility with ten children preschool and school aged. Both staff are not fully qualified. When the children transitioned inside, there was a total of 19 children supervised by 1 fully qualified teacher and four aides. This is an immidate risk to the health and safety of children in care. This is a repeat violation a civil penalty in the amount of $250.00 is issued.

Solid waste shall be stored, located and disposed of in a manner that will not transmit communicable diseases or odors, create a nuisance, or provide a breeding place or food source for insects or rodents. This requirement is not met as 1 The Facility Representative stated she will reach out to the landlord today and will come up with a plan to remove the trash from the premises. The Facility Representative is also required evidenced: LPA observed a large pile of trash in bags which has food waste, diapers and other disposed items. The pile of trash is located in front of the entrance of the facility. This is an immediate risk to the health and safety of children in care. 8 to come up with a plan for a permanent fix to ensure the trash is regularly removed from the premises. POC is due by 01/07/2022 LPA will return to correct the citation.

A teacher shall have completed, with passing grades, at least six postsecondary semester or equivalent quarter units of the education requirement specified in (c)(1) below, or shall have obtained a Child Development Assistance Permit issued by the California Commission on Teacher Credentialing. 1 Facility Representative will come up with a written plan of correction and will ensure children are left with qualified staff. LPA will return to clean the citation. This requirement is not met as evidenced: Staff #1 who was providing care to six preschool children is not teacher qualified. This is an immediate risk to the health and safety of children in care. 8 Written plan is due by POC date of 01/06/2022.

The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidenced: LPA observed a live cockroach in the microwave. This is a potential risk to the health and safety of children in care. 1 Facility Representative stated she will ensure to hire a pest company to ensure the facility is treated for pests by POC date of 02/04/2022. The facility will also be deep cleaned.

The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence shall be at least four feet high. This requirement is not met as evidenced: 1 Facility Representative will ensure to extend the fence to ensure the children's safety. LPA will return to correct the violation. The outdoor play area is not fully fenced as required. There is an opening of about two feet that is unfenced. This is a potential risk to the health and safety of children in care. 8
