At 9:15 AM, Licensing Staff observed two staff leaving the facility with 10 children from preschool and school age program. Licensing Staff observed the same staff supervising the children at Jack Davis park at 11:58 AM. This is an immediate risk to the health and safety of children in care. 8

This requirement is not met as 1 The Facility Representative stated she will reach out to the landlord today and will come up with a plan to remove the trash from the premises. The Facility Representative is also required evidenced: LPA observed a large pile of trash in bags which has food waste, diapers and other disposed items. The pile of trash is located in front of the entrance of the facility. This is an immediate risk to the health and safety of children in care. 8 to come up with a plan for a permanent fix to ensure the trash is regularly removed from the premises. POC is due by 01/07/2022 LPA will return to correct the citation.

The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidenced: LPA observed a live cockroach in the microwave. This is a potential risk to the health and safety of children in care. 1 Facility Representative stated she will ensure to hire a pest company to ensure the facility is treated for pests by POC date of 02/04/2022. The facility will also be deep cleaned.
