No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Director and facility representative Zina Rabinovich stated that since 09/15/2023, children has been escorted to and from the restroom by a staff members. Staff members have been informed that children This requirement is not met as evidence by: On 0915/2023, LPA Siao observed two school age children walking to the restroom located next to the playground during the inspection without visual supervision. This poses a potential health and safety risk to children in care. 8 cannot walk to and from the restroom without visual supervision.
Based on record review, the licensee did not comply with the section cited above in 1 out of 3 of the staff members immunization is not available for review. which poses/posed a potential health, safety or personal rights risk to persons in care.
