Based on LPA's inspection, Director interview, and parent interview, it was determined that C1's injury could have been prevented, if there had been appropriate cushioning underneath tunnel play structure, or if children had not been allowed to climb or jump from playstructure on the first place. 8 Director also stated that even though all staff have also been made aware of this new rule, they will all be asked to sign a date a written statement of their understanding to not allow children to climp/jump on top of the tunnel play structure.
Based on medication review, the licensee did not comply with the section cited above in 3 out of 4 child medications. One child's medication (C1) did not have a prescription label and the other two medications (C2 & C3) were expired which poses a potential health, safety or personal rights risk to persons in care.
