…Based upon staff interviews, C1, C10, and C12 were left without visual supervision in a bathroom on a field trip for less than two minutes which is a potential health, safety and personal rights risk to children in care. 8 The director will meet with all staff to reinforce active-supervision practices including supervision on fieldtrips and will provide the LPA with the training sign-in sheet upon completion by 02/23/26 .

Based on record review, the licensee did not comply with the section cited above in one out of 16 persons, as staff (S2) was not fingerprint cleared and associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA observation of epi-pens stored in inaccessible first aid kit in office, epi-pen for child C1 did not have a prescription label (or box) and epi-pen for child C2 expired 2/2023 which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation the licensee did not comply with the section cited above in that all classrooms had open trash cans with solid waste/food and did not have a tightfitting cover which poses a potential health, safety or personal rights risk to persons in care.
