101429(a)(2)(B) Responsibility for Providing Care and Supervision for Infants - Staff shall physically check on sleeping infant(s) every 15 minutes and document the...This requirement was not met as observed by: Facility representative Kinda Sancha states effective immediately,staff will perforrm physical checks every 15 minutes during nap time for all children under age of 24 months and submit copies to LPA by 3/6/26. Based on observations and a review of records, the facility failed to physically check on infants (who were napping in the preschool classrooms) every 15 minutes and maintain a sleep log which poses a potential risk to the health and safety and personal rights of children in care. 8
Facility Director Amitha Perusinghe will ensure that staff memebers are available on call to report to work if others are running late, in order to maintain proper ratios. LPA observed 2 staff (Kinda Sancha & Isaura Mayoral) with 9 infants for several minutes. Additional staff arrived late resulting in the facility being out of ratio by 1 infant.
