Based on interviews with children and staff, rodent activity was observed in classrooms #3 and #4 posing a potential risk to the health, safety, and personal rights of children in care. 8 The director provided the LPA with invoices documenting pest control services and corrective actions taken on 6/10/26
Based on observations, interviews and records reviewed, the facility commingled infants with preschool children. Infants were napping in the preschool classrooms #3 and #4 which poses a potential risk to the health and safety of those in care.
Based on LPA's review of facility records (sign in and sign out) and staff statements, some parents do not sign their children in and or out.
Based on observation there were 2 bird feeders filled with water located in the garden outside the classrooms. The garden is accessible to children. LPA observed children coming from the play area walking through the garden to their classrooms.
Based on observation, the facility front entry gate is broken and does not latch properly. LPA arrived at the facility and observed the front gate wide open. 8 Mrs. Perusinghe will send a photo to show that a temporary latch was installed on the gate today, 8/18/2025.
... 1 Director and Staff stated that the gate was fixed by the handyman over the holiday weekend. LPA observed upon arrival that the gate was functioning properly and lock code was also working. LPA provided clearance letter at time of inspection. Based upon staff and Director interviews, the front gate was not working properly which is a potential health, safety and personal rights risk to children in care.
