Based on observation, there were five infants in the front porition of the building. Two of the five infants were awake. This poses an immediate health and safety risk to children in care.
Based on record reviews and interview, S-1 completed courses Fall 2023. The final grade has not be posted yet. S-1 was alone with three infants, which poses a potential health and safety risk to children in care.
Based on observation and record review, the back room in the Cubs Dens building had nine (9) infants and two (2) staff. This poses an immediate risk to the health and safety to the children in care. 8 will also submit staff schedule to Licensing.
Based on record review, S-1 was in the infant program, but her fingerprints are associated to the preschool. This poses a potential health and safety risk to children in care.
Based on observation and interview, Licensee stated that she does not have anything in writing regarding training conducted with staff. LPA observed on 04/13/2023 that a staff used hand sanitizer in lieu of washing hands with soap and water after diaper change. Based on interview and record review, staff in the 12 months to 2 years old room does not do sleep checks for sleeping infants and the staff in the 12 months and under room does not check sleeping infants every 15 minutes. This poses a potential health and safety risk to children in care.
Based on observation, LPA observed that there was infant sleeping in the crib who had a pacifier. The pacifer had a leash attached to it, which poses a potential health and safety risk to children in care. There was also an infant who fell asleep the swing, who had a blanket on them.
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evident by 1 Licensee stated that she will remove mold from window and sink and send proof to Licensing. based on interviews and observations. LPAs observed mold on corner of window and under kitchen sink. This poses a potential risk to the health and safety of children in care. 8 Licensee fixed leak on the roof and will send invoice to Licensing.
