Based on record review, the facility did not ensure to maintain a level to be under 5.5 ppb at water fountains in classrooms. This poses a potential health, safety or personal rights risk to children in care. 8 Facility Representatives are looking into solution that was presented by the plumber. Site Manager will submit final decision to Licensing and retest of the affected faucets with the lead level under 5.5 ppb will be needed.

Based on interviews collected during today's visit, the involved staff had violated the children's personal rights which poses a potentital health and safety risk to children in care. 8 Facility representative will send the following to Licensing by due date 02/07/2025: 1. Internal investigation report to Licensing once it is finalized. 2. Personal Rights Meeting Agenda. 3. Staff Meeting attendance sheet 4. Proof of Involve staff trainings certificates.

Based on Interview, Regional Manager stated trainings have been conducted with staff. One of the Trainings Agendas included Children's Personal Rights. Based on interview collected during today's visit, the involved staff had violated the children's personal rights which poses a potentital health and safety risk to children in care. 8 Regional Manager will email to LPA proof of trainings provided to staff by due date 12/05/2023.
