... 1 Director held a classroom meeting to retrain staff on communication and supervision. Director also installed bells on the doors, started a new "teacher touch" rule for the children regarding doors and a number line to help with watching ratio. DIrector will have staff sign an agenda stating the received the training and send to LPA Lane via e-mail by 12/30/24. Based upon staff interviews and unusual incident report received from facility, C1 was left unsupervised for approximately one minute in the hallway and staff were unaware she had left the classroom which is a potential health, safety and personal rights risk to children in care.

… 1 Director stated she retrained staff regarding census counts during transitions and maintaining ratio. Director stated they will use a walkie talkie system for questions and assistance rather than staff leaving the classroom to bring a child to the office. Director stated she will provide proof of the staff training with agenda to LPA via e-mail by 3/10/23 along with staff attendance sign-in sheet. Based upon unusual incident report received and interview with Director and one staff member, a child was left alone for approximately 5-7 minutes on the playground which is a potential health, safety and personal rights risk to children in care.

… 1 Director stated she would hold a staff meeting training on food storage and preparation and have staff sign and date they have attended the training. Director will send signed statement via e-mail to LPA Lane by 8/8/22. Based upon duty call and incident report received, licensee did not ensure food was prepared and served in a safe and healthful manner which posed an immediate health and safety risk to children in care.
