(1) No child(ren) shall be left without the supervision of a teacher at any time This requirement was not met as observed by: 1 Facility Director Carlee Gosser provided LPA meeting agenda for the trainings facility conducted for staff on maintaining visual supervision at all time and implemented clear transition procedures and staff schedules. On 1/21/2026, staff failed to provide adequate supervision when a 3-year-old child was left unattended in the preschool play yard for approximately 1–2 minutes during a transition from outdoor play to the classroom. 8
