Director will provide a copy of the training agenda, sign in sheet, a LIC 855 Declaration summarizing the regulation and information learned in the video to LPA no later than 04/03/2024 via email.

101238(a)(1) Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidence by: 1 DIrector provided LPA with an invoice from 12/4/2023 showing that a pesticide company provided services while no children were present. Per Director, the next scheduled service will be conducted one month from 12/4/2023. Based on observations, interviews, Licensee did not comply with the section cited above. According to S1. S3 cockroaches were observed, which poses an potential health, safety or personal rights risk to persons in care. 8

101416.5 Staff-Infant Ratio: (b) There shall be a ratio of one teacher for every four infants in attendance. This standard was not met based on evidence obtained through the interviews, revirew documents observations. Ms. Jaqueline Lopez, designated person in chage, provided a declaration (lic855 form), that she understand the Staff-Infant Ratio and stated that she will schedule LPA reviewed Sign in & out and staff timesheet for the day 5/12 and observed when 2 additional teachers arrived at 8:00am. From 6:30am to 8:00am, facility have 2 teachers with 12 infants in care. Faciltiy was over ratio. This poses an immediate Health and Safety risk to children in care. 8 more teachers, for ealy hours and will make sure the facility will be in compliance with the Staff-Infant Ratio. Ms. Lopez agreed to have TSP, LPA will referral to TSP team to contact the facility as soon as possible.

On 5/11/22 LPA reviewed sign Per Director Assistant, teachers will be schedule to start ealry and temporary agency will be contact for hire teachers. Director assistant provided a declaration regarding her plan of correction how to maintain the staff-infant ratio. . sign in & out and timesheets for the days 5/10 and 5/11/22 and observations. Faciltiy was over ratio, one teacher was supervisint more than 4 infants. This poses an immediate Health and Safety risk to children in care. 8 Per Director, she understand that regulation Staff-Infant Ratio, that shall be a ratio of one teacher for every four infants in attendance. LPA provided copy of title22 101416/5 Staff infant ratio.
