Based on observation, interview and record review, 3 of 10 children enrolled were not singed in, which poses a potential Health or Safety, or personal rights risk to persons in care. Director will provide LPA copy of staff signing off on receipt of memo.
101229 (a)(1) Responsibility for Providing Care and Supervision. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Licensee will ensure that children are supervised at all times. Licensee will meet with staff on 12/8/21 and submit a plan of correction to the department. Licensee submitted a plan of correction to the department. During the inspection on 12/7/21, LPA Wheatley observed three (3) children playing in the outdoor play area unsupervised by staff. LPA called for assistance from staff who were outside however in a different area of the yard. This poses an immediate health and safety risk. 8
Based on a file reviews of staff, both S2 and S5 do not have the required couses to be fully qualified teachers. S2 is a qualified teaher assistant and S5 is qualified to volunteer.
Based on observations S1 was screaming at 2 children that had wondered into an off limits area of the yard where there was no direct superviosion.
Based on file reviews of personnel, no employee fies contained signed LIC9052 which poses a potential Health or Safety, or personal rights risk to persons in care.
Based on observation the insdie facility has a lot of flies. Director states that this might be from gunea pigs that sleep over night in a cage inside the facility.
