This requirement is not met as evidence by: 1 Director returned to oversee faciilty as of October 2, 2023. Based on observations and interviews, interim director was not reported to the the department for the 5 months that the director was not present, which poses a potential health, safety or personal rights risk to persons in care. 8
101238(a) Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidence by: 1 Per Director, Owner has been contacted and repairs will be made on or before 3/8/2024. Visit will be scheduled to observe repairs made. Based on observations,facility is in unsafe and in need of repair, which poses a potential health, safety or personal rights risk to persons in care. 8
Lead water testing samples taken on 11/03/2022 revealed that levels of lead 1 Per Assistant Director, facility will either replace fixtures or submit a plan on how to provide potable water to children in care. Director will provide plan of correction and any update as necessary by 12/30/2022. were beyond the allowable amount deemed by the State of California. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care.
Based on LPA interviews with staff and children, who disclosed that there was an ongoing pest/cockroach issue, which poses immediate Health and Safety risks to children in care.

Based on records review. LPA reviewed 07 files they are incompleted.
