Based on interviews of the staff and the child, the facility failed to provide safe, healthful, and comfortable accommodations and the child did not like the dog jumping on them and during a later time when the chld noticed the dog behind a gate, became emotional. ensure the safety and well-being of the children. The Director will provide a copy of the training agenda and sign-in sheet to LPA no later than 11/14/2023.
