101429 Responsibility for Providing Care and Supervision for Infants (a) In addition to Section 101229 ...(1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This was not met by evidence of Director notified the Regional Office via an UIR on 09/12/22 indicating an all staff meeting was held reviewing supervision procedures. Per interview with director on 09/21/22, procedures have been established and staff are using the face-to-name procedures whenever a child is in the classroom. 3 staff failed to supervise an infant child (1 year old) by leaving him alone in the classroom during transition time at or around 4:07 p.m. for approximately 2 minutes. This is an immediate risk to the safety of children in care. 8 Deficiency is being cleared during today's visit as LPA was able to verify the procedures are being followed. A $500.00 civil penalty is being issued today due to absence of supervision.

Robin Cruz is pending fingerprint clearance and has been working at the facility since 1 Director immediately removed Robin Cruz, from facility. Per Director, will resume employment when Robin is fingerprint cleared and associated. Per Director, will provide proof of fingerprint clearance to LPA Gontarek via email. 2/14/22, and did not obtain a criminal record clearance prior to initial presence in the facility. LPA Gontarek reviewed Licensing Information System (LIS) for employee status, and LIS stated "Overall Status- Pending." This is an immediate health and safety and personal rights risk to the children in care. Civil penalty of $500 was assessed.

LPAs observed 9 infants in the Toddler 'A' classroom with 2 staff. The classroom was over ratio by one child. LPAs observed Director remove one child from the Toddler 'A' room, and place child into Toddler 1 Per Director, will have an office staff member step into classroom when a classroom is short-staffed. Per Director, will use staffing agency to meet teacher-child ratio. Per Director, will submit registration confirmation from staffing agency to LPA via email. Child Care Careers (714) 634-2947 'B' room. This is an immediate health and safety risk to the children in care. 8 Per Director, will use staff from "sister school" in the Torrence location. Per Director, will have a staff training to discuss importance of personal rights and ratio. Per Director, will provide copy of Agenda Notes and Staff signatures via email after trainiing is conducted.
