Based on interviews, the Licensee did not comply with the section cited above in that they did not ensure that each infant's diapers are being changed as often as necessary to ensure that the infant is clean and dry at all times, which poses an immediate health, safety, or personal rights risk to persons in care. 8 observed partial on-site correction as Director spoke with staff regarding diapering procedures & the importance of ensuring that children are clean and dry at all times.

There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. 1 LPA Observed staff being back in ratio before leaving. As a plan of correction, assistant agreed on an informal conference with Child Care Licensing (CCL) as well as a written statement stating how to prevent being out of ratio in the future. An informal Conference date will be set by the plan of correction due date. Based on observation, staff interviews and record review, the facility did not comply with 101216.3 TEACHER-CHILD RATIO. LPA observed staff (S1) providing care for nineteen (19) children alone which posses an immediate health, safety or personal rights risk to persons in care. 8 In addition to the wriiten plan, an informal conference was completed on 4/4/2022 and the written Plan of Correction was received on March 28, 2022

This requirement is not met as evidence by: 1 Director will submit a written plan of correction stating measures to be taken in order to be in compliance with Title 22 Regulations and avoid any Personal Rights violations by 10/11/2021 via fax (805) 685-1820 or email to francisco.pedroza@dss.ca.gov Teacher was observed guiding a child's head down and pulling their legs outward forcing them to lay down on the mat. Teacher's employment was discontinued. This poses as a potential Health and Safety risk to clients / children in care. 8
